Documents Required

We request you to submit the following documents to register a death claimNotification to an insurance company that payment of an amount is due under the terms of the policy.:

• Original policy documents
• Original / attested Death Certificate issued by municipal authorities

• Electronic Funds Transfer - mandate form to be completed by the nomineeA person or firm into whose name the policy is transferred in order to facilitate transactions, while leaving the customer as the actual owner. with the copy of cancelled cheque
• Attested Photo ID proof of nominees bearing his/her photograph and signature e.g. PAN Card, driving license, passport, etc.

In addition to the above, you are requested to submit the following documents as per the cause of death:

• Medical
  • Attending physician's statement - 'Form C' to be completed by all Doctor(s) and hospital authorities who attended to the life assuredThe person covered by a life insurance policy. The insurance company pays out on the death of the policyholder. during the last illness or accident
  • Patient admission sheet and discharge / death summary, investigations and treatment records from hospital(s) admitted and postmortem report (if conducted)
• Accidental
  • All records from hospital(s) admitted at the time of death including postmortem / autopsy report and viscera and histopathology report
  • Police records:
    • First Information Report (FIR)
    • Panchanama
    • Final Police Investigation Report (FPIR)
  • Driving license of the life assuredThe person covered by a life insurance policy. The insurance company pays out on the death of the policyholder. (in case road accidents only)

Group Term Life Insurance Plan (EDLI / Employer- Employee / Gratuity)

Affinity

• Claimant statement - 'Form Affinity' to be completed, signed, stamped and witnessed by the policyholder
• Original / attested copy of Death Certificate issued by municipal authorities
• Certificate of insurance
• Declaration of health
• Attending physician's statement - Form C to be completed by life assuredThe person covered by a life insurance policy.   • Attested Photo ID proof of nominees bearing his/her photograph and signature e.g. PAN Card, driving license, passport, etc.
• Additional documents for Affinity-Credit life
  • EFT mandate form to be completed by policyholder along with the copy of the cancelled cheque wherein the name of the account holder is printed
  • Schedule of loan outstanding as on the date of the insured eventThe event that has been covered in the insurance policy.
