You can receive the  maturity claim for your annuity policy easily by submitting the mandatory documents at the nearest Axis Max Life Office. Here is the list of documents:

• Duly filled Annuity Settlement Form 
• A cancelled cheque or copy of passbook (with pre-printed name and bank account number) for pay-out through NEFT 
• Original Policy Pack or lost policy affidavit in case the policyholder has misplaced the policy document 
• A self-attested photo ID proof
• A valid proof of address
• Copy of the PAN Card
• Income Proof
• In case of joint life annuity: - a self-attested ID proof, and - a copy of the PAN card of the joint life annuitant
